S A F E T Y   B O N U S
  $120.00 Base Safety Bonus/Standard Driver Qualification
(No Accidents, No Personal Injuries, No negative Road Patrols, no founded negative HMD’s and No Out of Service Violations on DOT Roadside Inspections.) 
  $65.00 If driver has qualified for the Base Safety Bonus and he/she has successfully turned in all DVIR’s, Hours of Service Logs, Monthly Maintenance Reports, an additional $65.00 will be awarded.
Any driver with one or more critical violations will not be awarded this portion of the bonus. Critical Violations are as follows:
  1. Hours of Service Violation
  2. Log Falsification
  3. Speeding Violation
  $65.00 If driver has qualified for Base safety Bonus and his/her
terminal is at or under their preventable accident frequency goal, an additional $65.00 will be awarded.
(Terminal Preventable Frequency is calculated YTD.)
